Denaro's notoriety escalated dramatically on April 12, 2001, when he graced the cover of L'Espresso magazine, which boldly declared him the new Mafia boss. His criminal activities led him to become a fugitive in 1993, placing him on the Italian Ministry of the Interior's most wanted list. By 2010, Forbes recognized him as one of the ten most wanted and powerful criminals globally, solidifying his reputation as a formidable figure in organized crime.

With the deaths of his predecessors, Provenzano in 2016 and Salvatore Riina in 2017, Messina Denaro was viewed as the unchallenged boss of all bosses within the Mafia.
