In two thousand six, Petrov transitioned to the Premier League, signing with Aston Villa under the guidance of his former manager, Martin O'Neill. He quickly rose to prominence at Villa Park, eventually becoming the club captain. His contributions to the team were recognized in two thousand thirteen when he was inducted into the Aston Villa Hall of Fame, having made two hundred nineteen competitive appearances for the club.

On the international stage, Petrov holds the record as Bulgaria's all-time most-capped player, with one hundred five appearances for the national team. However, his career faced a significant challenge in March two thousand twelve when he was diagnosed with acute leukaemia, leading him to pause his football journey for treatment, which ultimately proved successful.
