As the elder daughter of Prince Tomohito of Mikasa and Princess Tomohito of Mikasa (Nobuko), she holds a unique position within the royal family, being a paternal second cousin to Emperor Naruhito.

Her family connections extend beyond the imperial lineage; she is the niece of Japan's ninety-second prime minister, Tarō Asō, and a great-niece of the renowned author and literary critic Ken'ichi Yoshida. Additionally, she is a great-granddaughter of Shigeru Yoshida, who served as Japan's forty-fifth prime minister.

In September twenty twenty-five, Princess Akiko made history by becoming the head of the Mikasa-no-miya branch of the imperial family. This significant role marks her as the first woman born a princess to succeed as head of a branch of the imperial family since Princess Sumiko, highlighting her influential position in contemporary Japanese society.
