As the eldest son of Takahito, Prince Mikasa, and Yuriko, Princess Mikasa, he held a significant position within the Japanese royal family. His lineage connected him as a first cousin to Emperor Akihito, placing him sixth in the line of succession to the Japanese throne.

Throughout his life, Prince Tomohito was recognized not only for his royal heritage but also for his unique appearance. He was the first member of the Imperial House to sport a full beard since Emperor Meiji, which earned him the affectionate nickname of the 'Bearded Prince' (ヒゲの殿下 Hige no Denka).
